By signing below, I (the "Client") authorize Integrity Payroll Services LLC (the "Provider") to perform the payroll services selected above. The Client agrees to:
1. Provide accurate and timely employee data, including hours worked, salary changes, and new hire information. 2. Maintain sufficient funds in the designated bank account to cover payroll and tax obligations. 3. Review all payroll reports promptly and notify the Provider of any errors within 48 hours. 4. Pay all service fees as outlined in the fee schedule provided separately. The Provider agrees to process payroll accurately and file all applicable tax returns on time. This agreement remains in effect until terminated by either party with 30 days written notice. This agreement is governed by the laws of the Commonwealth of Pennsylvania.
